Adoption Agreement

1. Transfer of Ownership: I understand that this dog is being adopted into my care, and I accept full responsibility for the dog’s safety, health, and well-being as a companion and family member.
2. Humane Treatment: I agree to treat the dog with compassion, provide a safe home, and protect them from all forms of abuse, neglect, or mistreatment.
3. Food and Water: I will feed the dog a balanced diet at least twice a day and ensure access to fresh, clean water at all times.
4. Living Conditions: The dog will live primarily inside my home and will never be kept chained, tethered, or left outside unattended. The dog will not be confined to a yard, patio, balcony, or garage for extended periods.
5. Cosmetic Procedures: I will not perform or authorize any elective cosmetic surgeries such as ear cropping or tail docking.
6. Veterinary Care: I will take the dog to a licensed veterinarian within fourteen (14) days of adoption for a wellness exam and continue with annual checkups, vaccines, parasite prevention, and prompt treatment for illness or injury.
7. Licensing & Legal Compliance: I will obtain all licenses, tags, and microchip registrations as required by local and state law.
8. Training & Behavior: I agree to train the dog using only positive reinforcement methods and will seek professional help if behavioral issues arise.
9. Rehoming or Transfer: I understand that this dog is being adopted for my household only and may not be sold, gifted, or surrendered to another party or shelter without written consent from Good Omen Dog Rescue (GODR). If I can no longer keep the dog, I will contact GODR immediately and continue to care for the dog until rehoming arrangements are made, allowing a minimum of two (2) weeks. I agree to cover any costs associated with returning the dog to GODR if required.
10. Lost, Stolen, or Deceased Dog: I will notify GODR immediately if the dog is lost, stolen, or passes away, and I will update GODR promptly if my contact information changes.
11. Dog’s Background: I understand that GODR has disclosed all known information about the dog’s health and temperament, but makes no guarantees regarding breed, health condition, or future behavior.
12. Medical Assistance Policy: I understand that GODR may, at its sole discretion, offer limited financial assistance for veterinary care in extraordinary circumstances, but such support is not guaranteed.
13. Adoption Donation: I understand that adoption donations are non-refundable and help support GODR’s rescue, medical, and care efforts. Within the first fourteen (14) days, GODR may, at its discretion, help find a more suitable match if needed.
14. Truthfulness: I affirm that all information I provided in my adoption application is accurate and that no one in my household has ever been convicted of animal cruelty, neglect, or abandonment.
15. Adoption Finalization: I understand that the adoption is finalized only after payment of the adoption donation, submission of this signed agreement, and physical transfer of the dog.
16. Right of Reclamation: I acknowledge that GODR reserves the right to reclaim the dog if any terms of this contract are violated. GODR will make reasonable attempts to notify me in writing before reclaiming the dog, but may act immediately if the dog’s safety is at risk.
17. Release of Liability: I accept full responsibility for the dog’s care and behavior. I release Good Omen Dog Rescue, its officers, employees, and volunteers from any liability related to the dog after adoption.
18. Media Consent: I grant permission for GODR to use photos or videos of me and the adopted dog for promotional or educational purposes without compensation.
